This 401(k) provider is a small company that I can't find any way to contact directly. The one contact I have claims to have sent the withdrawal check to Vanguard, but Vanguard never received it. When I sign in to my old 401(k) provider the account shows a $0 balance and is marked as a withdrawal that says "terminated". I had over $25,000 in the account that has disappeared. When I reach out to my work contact they refuse to do anything and tell me just to keep waiting. What do I do?

Edit: It has been almost 6 months. 4 months of which were trying to get my employer to provide proof to termination to the 401(k) provider and 2 months since I have made the withdrawal request.
