We visited the ER on a vacation back in 2019. Everything was out of network and we, very stupidly, used the insurance provided payment portals to pay some of the bills. In the mix up, we were sent to collections agencies after paying and being reimbursed multiple times. We finally figured it all out after months of back and forth.

Now, an already paid bill has gone to another collection agency. I have checked the statement online and it shows a zero balance (because we literally already paid it).

The letter says to submit a written request to dispute within 30 days. I obviously plan on disputing this but is there anything specific I should have in hand and prepared? Is there any risk in calling them directly vs. sending a written request back? I know the magic word is "dispute" but what else can I say?
