Sending certified mail for a debt in my maiden name.

First - I am in the process of following the wiki for debt collection.

I am in the process of drafting the letter to request all the documentation. The debt is listed in my maiden name. When signing the letter (not physically just my name at the end) how should I format it?

Jane Smith

Jane Tree

Jane Tree (Smith)

or some other way?
