I had heart surgery about 3 months ago and the total came out to $790,000 and change. At the time, I was 25 years old and covered by my dad's insurance through his work (Primary) and my stepmom's insurance through her work (Secondary).

Over the past 9 months (long before the most recent surgery) my Primary insurance provider has been refusing to pay smaller claims because they are claiming that they are not actually my Primary insurance (they are). They are trying to pass the buck off to everyone else, including my stepmom's insurance and my biological mom's insurance, even though I haven't been covered by my biological mom in months (since she was let go from her job). This initially began due to an error that my dad made on an insurance form, which I have been trying to correct now for months (he indicated that my mom's insurance was good through 2019, but she ended up being let go way before that).

My hospital is threatening to classify the bill as self-pay if someone doesn't cover the cost soon. What can I do? I've spent hours on the phone with the Primary insurance, have emailed and faxed numerous Coordination of Benefits forms, all of which seem to get "lost" and never make it to the company's records. I have to re-explain the situation with every new representative I speak to. I ask to have the claims escalated to a manager and I am told they are busy and they will call me back. They don't.

I am at a loss for how to move forward.
