Following the overwhelming advice of the community, I emailed HR explaining the situation, and they investigated quickly. It turns out that the bonus was "supposed to" have the 1 year claw-back provision on it, but they forgot to send me the paperwork when the bonus was awarded to me. Since I never received the document, I was genuinely unaware of this. Fortunately, they agreed it was mistake on their end and let me keep the bonus. Pure class from them.
The exit interview went smoothly and they wished me luck on my new job. Overall, I would say it was the best-case scenario for me, having been able to keep the bonus and leave on good terms.
