I received a 1099-NEC in the mail from Amazon for "Amazon Flex" in the amount of almost $14k for the 2023 tax year. I don't sell on or work for Amazon and have never received payments from Amazon for anything.

The form has my correct first, middle, and last name and my correct last 4 SSN digits. The address on the form is my city, state, zip, and street name however the street number is fake (looking it up in google maps doesn't point to a valid address). I assume the post office forwarded it to me when they tried to deliver to the fake address.

I'm trying to figure out what I need to do:

  • I already had all my credit reports frozen (did it years ago)
  • I pulled a new credit report and it's clean.
  • There's no Amazon contact info on the form.
  • I did some googling and the IRS says to submit form 14039 if someone files a fraudulent tax return under your SSN but I haven't found anything saying what to do just for a fraudulent 1099 without a fraudulent return being filed.
  • The IRS also says if you suspect you're a victim of identity theft to request "An Identity Protection PIN" but the webpage for doing so says "Service Outage: November 18 to January".

Any advice?
