I got an HMO insurance. If I contacted my doctor office staff on the phone, and I asked for some lab work order to take with me to the lab, is that considered an office visit co pay? I used to ask it in the past and never got charged for it, I thought calling them on the phone and asking for things, as long as they don't tell you that you need to come in to see the doctor is ok. I filled a grievance with the insurance and they tell me that I should pay referring to my EOC which says for my office visits the following: " Visit to Physician, Physician Assistant or Nurse Practitioner at a contracting Physician Group. .... $xx co pay"
Given that I didn't really visit the my doctor office or his office staff, but I might have spoken with them on the phone, is that considered that I need to pay it? Shouldn't they tell/warn me upfront that I should pay for it?
