My wife and I work for the same employer. We are on separate health insurance policies, I'm on my own and she's on a policy with our infant daughter. We save ourselves (and our employer) hundreds of dollars a year doing insurance this way vs being on a family plan.

Today my wife got a call from HR telling her that she and our daughter are no longer covered and that they show the policy was canceled last week. They basically said they have no idea how this could've happened (or who canceled it), and are working with the insurance company to get it resolved.

Basically, I'm wondering if there's anything we should do in the meantime? We mainly just want to all be insured again, but also are curious how this could've happened so we can make sure we don't end up in this situation again.

We are both still employed full time. Nothing has changed that would disqualify coverage, and we didn't receive any communication from the insurer about possible fraud.

Has anyone here had a similar experience?
